The utility of perfusion/diffusion mismatch as a criteria for arterial intervention has been demonstrated by multiple trials (DEFUSE, DEFUSE2) and is endorsed in the 2009 AHA stroke guidelines (Class I, LOE A). Our practice has also found value in the application of perfusion to assess vertebrobasilar insufficiency.
